I have seen issues/PRs go AWOL on some of my repositories before as well. I believe that it occurs when the creator of the issue/PR has been banned by GitHub. Anything a banned user has created (PR, issues, comments, etc) are deleted from the service. They seem to ban people if they think they are generating spam, are being abusive to other users, or have otherwise violated their Term of Services (such as creating more than 1 free account on the service).
