I think it is, or was, true: A pull-up on the DO cannot be omited, or some cards will fail initialization process. I don't know whether they ever (going back to '97) actually used open collectors. Modern SD cards use tristate drivers, of course....Also, I think the author is wrong regarding SD-MMC...
