POWER_OFF_ON_HALT=0 will go to the VPU sleep state whilst RP1 is still in reset so I would expect that the PWM signal would be low and fan running.
It might be possible to drive this pin without booting the RP1 firmware, I'll check tomorrow, possibly something in rpi-update if it's the simple change that I think it is.
